Student Services Department handles all new student (transfer) registration. 
Steps to Register: 

1. Residency Verification - Form 
2. Provide any past assessment scores (MAP, EXPLORE, etc.)
3. Sign Release of Information - Form   
4. Home Language Survey - Form 
5. Counselor Appointment - parent and student meet with school counselor to develop class schedule 
6. Business Office - school fees, fee waiver, iPad insurance, etc.

1.  Birth Certificate 
2.  Enrollment Form
3.  Record of Vaccinations 
4.  Dental Examination Form 
5.  State of IL Certificate of Child Health Examination Form
6.  District 229 Medication Authorization Form (completed by IL Physician) 
7.  IL Eye Examination Form 
8.  Sports/PE Restrictions
